A patient with an infected foot

We have a 26 year old man coming in with an infected swollen right leg and having a fever. He is a victim of the earthquake with the last 2 toes amputated. Problem now is he has an open wound in his big and second toes, being neglected for a long time without medical treatment. He is actually mentally confused on admission. I did a debridement for him, only to find out that his big toe wound was necrotic and lots of pus coming from within the foot but the most disgusting thing was there were many maggot worms inside his second toe wound which I need to grab them all out. His condition is a bit better after several treatments to his wound, the fever starting to subside, but there’s still thin pus irrigated out from within the foot every time I do the cleaning. I hope his infection will be subsided, otherwise I may have to think of amputating his foot to control the infection, but I hope he will never need.
